How do I find your office?
Our office is located within the Pathways Wellness Center, Suite 308. We are in the white building (524 Milwaukee St) and can be found across the street from Blue Collar Coffee Co.
Do you accept insurance?
At Alleviate Chiropractic and Wellness, we have made the decision to prioritize your health and lifestyle goals over the limitations set by insurance providers. We believe that healthcare should be personalized and focused on your specific needs, not constrained by the one-size-fits-all guidelines imposed by insurance companies. Our mission is to work for YOU, not the insurance companies.
Please note that we operate on a self-payment basis, and payment is due at the time of service. We accept credit/debit cards and FSA/HSA accounts. While we do not directly accept insurance, we are happy to provide superbills that you can submit to your insurance provider for potential reimbursement. However, please be aware that reimbursement is not guaranteed.
What should I wear?
Please come to your appointments in athletic clothing. If you have a hip, knee, or lower extremity injury please bring shorts.
What will my treatment include?
Treatments vary from person to person based off your individual needs. Every treatment that we provide is covered under your appointment with no additional costs.
If I already have X-Rays or MRIs, should I bring copies?
Absolutely! We encourage bringing in any imaging and reports that can help with your treatment.
Do you offer treatment packages or savings options?
Yes! We offer careplan packages to make it easier and more affordable for you to commit to your health journey. Our packages are designed to provide long-term benefits by offering upfront savings while ensuring you get the consistent care you need for optimal results.
